Alright this was on reddit, I suspect it has to do with a potential fix to Meld not working if you cancel attack.
Here's the match ID with the timestamp dota2://matchid=28825008&matchtime=1231
It happens right after, go to bot lane where TA is. She melds, then attack a creep out of meld but the creep dies before the projectiles hit. Then she runs around with Meld buff still on. Several seconds later, she attacks a creep and get the meld bonus attack off so it's not just a visual bug either. I went into practice and found an easy repro:
Pick TA, level Meld and Psi blades so you have more time before projectile reaches target. Spawn puck and level phase shift or any hero and give him a cyclone. Go into meld, attack, then avoid that attack with cyclone/phaseshift/whatever. You can now move while melded as TA. The key here is as long as the attack doesn't land for whatever reason, you stay melded but can now move.

This bug is almost fully fixed. Disjointing the Meld attack via invisibility is the only thing which still causes this bug.
Every other way to evade that attack doesn't cause it anymore, so only invisibility is left to fix.
