There is an issue with the item slot that this particular item should be assigned to.
In the attached image prisoners anchor.jpg shows that the item is currently taking up the "armor" slot outlined in red. This is the slot that all head items are also assigned to. This is shown in the image prisoners anchor 2.jpg where the Cranial Claptrap cosmetic is assigned to the "armor" slot. These two items should not conflict as the anchor has no relevance to the general head area. In the prisoners anchor 3.jpg it shows the intended aesthetic where the head ornament is not removed. However, again referring to "prisoners anchor.jpg" it clearly removes any head cosmetic as it uses the same slot (with the unsightly side effect that it also removes the original head ornament).
The solution is to reassign the genuine prisoner's anchor cosmetic to the item slot that the arrow directs to in "prisoners anchor 2.jpg". The empty slot that the arrow directs to currently has no cosmetic item assigned and that is where this anchor should be placed. Although the naming convention probably hints that all existing head items should have been assigned to the "head-body" slot with the anchor meant for the "armor" slot.
